New research: do varicose veins raise the risk of a serious central-vision problem (CSCR)?

A large new study in the British Journal of Ophthalmology found that people with varicose veins were about twice as likely to be diagnosed with central serous chorioretinopathy (CSCR), a condition where fluid collects under the central retina and blurs or distorts vision. This shows an association, not proof that varicose veins cause it. If you have varicose veins, it is a good reason to keep a regular eye check and to notice any sudden central blur.

What the study did

Researchers at Stanford and Mass Eye and Ear used the TriNetX health-records network. They matched 621,513 people with varicose veins one-to-one with people without them, by age, sex and ethnicity, after excluding anyone who already had CSCR. Then they compared how many in each group were later diagnosed with CSCR or with a related fluid problem called serous pigment epithelial detachment.

What it found

  • Varicose veins were linked with CSCR: risk ratio 2.16 (95% CI 1.86 to 2.52)
  • They were also linked with serous PED: risk ratio 1.97
  • After excluding steroid users and matching for high blood pressure, sleep apnoea and healthcare use, the link remained: risk ratio 1.83 for CSCR
  • Among people with varicose veins, those who also had CSCR did not have a significantly different rate of deep vein thrombosis or pulmonary embolism

The authors suggest a shared vascular mechanism, in line with earlier genetic work. Both conditions involve abnormal, enlarged veins.

What CSCR is

CSCR happens when fluid leaks under the macula, the central part of the retina. People notice a blurred or dim patch in the centre, straight lines looking bent, objects looking smaller, or colours looking washed out, usually in one eye. It is more common in men aged 30 to 50 and is linked with stress and with steroid medicines, including some creams, injections and tablets. Many cases settle by themselves, but some need treatment from a retina specialist.

Limits of the study

  • It is a records-based study, so it cannot prove cause and effect
  • The data come mostly from health systems in other countries, and Indian patients may differ
  • Records only capture people who were diagnosed, so mild CSCR may be missed
  • A risk ratio of about two still means CSCR is uncommon in absolute terms: most people with varicose veins will never develop it

What it means for people in Lucknow and India

Varicose veins are common among people who stand for long hours, such as shopkeepers, teachers, traffic and factory workers. This study does not mean you should worry, but it is a reminder that eye health and general health are connected. Practical steps:

  1. Know the symptoms: sudden central blur, a dark or grey patch, or wavy straight lines in one eye
  2. Check one eye at a time, covering the other, while looking at a door frame or a tiled wall
  3. Tell your doctor about steroid medicines, including creams and ointments, because they are a known trigger
  4. Do not delay if central vision suddenly changes: get a same-day check with an eye specialist
  5. Keep a regular eye test, especially if you also have diabetes or high blood pressure

Frequently asked questions

Do varicose veins cause eye problems?

This study found they are associated with CSCR, but it does not prove they cause it. Most people with varicose veins have no eye problem.

What are the first signs of CSCR?

Usually a blurred or dim spot in the centre of vision in one eye, straight lines looking wavy, or things looking smaller or less colourful.

Does stress cause CSCR?

Stress and steroid medicines are both linked with CSCR in earlier research. Avoid steroids unless your doctor prescribes them.

Can glasses fix CSCR blur?

No. Glasses correct focusing errors, not fluid under the retina. A proper eye test helps tell the two apart.

Should I get my eyes checked if I have varicose veins?

A regular yearly eye test is sensible for everyone, and more so if you notice any change in central vision.
